Overview
An opportunity has arisen for an Occupational Therapist to join a multidisciplinary team within a rehabilitation setting supporting adults transitioning from acute hospital care. The service focuses on promoting independence, functional ability, and safe discharge planning.
Key Responsibilities- Complete functional, cognitive, and ADL assessments
- Deliver individualised rehabilitation programmes
- Support discharge planning and home assessments
- Recommend assistive equipment and environmental adaptations
- Work closely with the MDT to support patient-centred care and outcomes
- Qualification in Occupational Therapy
- CORU registration
- Experience in rehabilitation, acute hospital, or older persons services
- Strong understanding of functional independence and discharge pathways
