Client Care Co-ordinator, Co Galway

Our established Homecare Client is seeking applications for the role of Homecare Assessor. Our client’s healthcare team is focused on assisting their clients to live as independently as possible in the comfort of their own homes.

The assessor conducts comprehensive assessments, develops care plans, and collaborates with the interdisciplinary Team to ensure the provision of high-quality care in a homecare setting.

To be considered for the role, the following criteria apply:

• Bachelor’s degree in nursing or a related healthcare field.
• A higher degree or certification in home healthcare or clinical assessment is preferred
• Minimum of 2 years of clinical experience in a hospital or community healthcare setting, with a focus on Client assessment and care planning.
• Strong understanding of medical conditions, treatment modalities, and care protocols relevant to homecare.
• Familiarity with home healthcare regulations and HSE systems is advantageous.
• Excellent verbal and written communication skills
• Strong analytical and problem-solving abilities.
• Demonstrated ability to prioritize tasks, manage multiple assignments simultaneously,
• Proven ability to work effectively within a multidisciplinary Team
• Proficiency in using electronic health record (EHR) systems and other relevant software.
• Full Clean Driving Licence and access to a car.
• Eligible to work in Ireland.
