Job Role
As a regional Care Manager with Komfort Kare, you will take full ownership of your area, liaising directly with clients and support workers to ensure full coverage of schedules. If you are a strong Care Scheduler, Care Coordinator, or Supervisor looking to make the next step, and if you have good communication skills and dedication to helping vulnerable people, we would like to hear from you!
The Care Manager will assist with completing the administration tasks that go along with rostering and scheduling care for their area. This is an excellent opportunity for a dedicated candidate to take their first step into a management role.The areas this job will cover are: Donegal, Sligo, Leitrim, Mayo, Galway, Roscommon
The ideal candidate will be capable of managing all aspects of client schedules. They will take responsibility and accountability, possess excellent communication skills, and be dedicated to providing the highest quality of home care to those most in need.
Job DescriptionCreating and updating personalized care plans for patients.
Collaborating with patients' families, friends, and social supports in developing treatment plans.
Identifying and recruiting high-quality healthcare providers.
Interacting with healthcare providers to facilitate patients' treatments.
Ensuring patients’ treatment requirements are met.
Suggesting alternative treatment plans when patients' service requests do not meet medical necessity criteria.
Instructing and educating patients/clients on procedures, healthcare provider instructions, and referrals.
Linking patients/clients to social services programs and entitlements such as transportation assistance and translation services.
Conducting regular follow-ups with patients to evaluate progress, promote continuity of care, and ensure improved health outcomes.
Maintaining records of case management activities.
Proficient with healthcare management systems and Microsoft Office Word and Excel.
Outstanding communication skills, both written and verbal.
Excellent organizational skills.
Ability to travel locally to assist carers if needed.
Schedule
Monday to Friday - On-call shifts after office hours
Rotation on weekends as Support Manager for On-Call
9 am to 5 pm
Based in SantryOffice
Salary Range
DOE and qualification
