Care Co-ordinator – Older Persons Services
FRS are recruiting on behalf of our client for an experienced Care Co-ordinator to support the delivery of high-quality home care services.
This role involves managing care rotas, allocating care staff efficiently, and ensuring customers receive safe, consistent, and person-centred care in their own homes. You will respond to schedule changes, arrange cover when required, and communicate effectively with both customers and the care team.
Key responsibilities include:
- Coordinating and allocating care staff in line with care plans
- Managing rotas and arranging cover for sickness and holidays
- Processing new referrals and schedule changes
- Using care management systems to match staff skills to customer needs
- Communicating changes clearly with customers and staff
- Maintaining accurate records and confidentiality
- Participating in an on-call rota to ensure continuity of care
