Social Worker

We are currently seeking a dedicated Social Worker to join a dynamic multidisciplinary team, providing essential advocacy and support services to individuals and their families. This role involves managing a caseload of varying complexity while ensuring that each person receives the highest level of care and access to the services they need.

Key Responsibilities:

• Manage a diverse caseload, addressing both individual and family needs.
• Advocate for the rights and entitlements of individuals, ensuring they are informed and supported.
• Facilitate advocacy sessions, guiding individuals through self-advocacy and group advocacy initiatives.
• Identify and address unmet needs, working closely with the team to develop appropriate solutions.
• Participate in care planning and multidisciplinary meetings, collaborating with staff and clinicians to enhance support services.
• Provide safeguarding support, assisting designated officers in protecting vulnerable individuals.
• Contribute to funding applications and reports, ensuring accurate documentation and compliance with best practices.
• Maintain professional standards in line with current legislation and organizational policies.

Requirements:

• Recognized Social Work qualification (NQSW/CQSW) or equivalent.
• CORU registration (or in the process of registering).
• Minimum one year of professional experience in social work.
• Experience working within a multidisciplinary team.
• Strong IT skills for documentation and reporting.
• Full, clean driving license and access to a car.
